My travel out of the country has been limited to Mexico and the Bahamas. Neither required much more than birth certificate and picture ID (At least when we traveled 5 yrs ago.) Anyway my knowledge of travel documents is somewhat limited. One of our banks is in a university town and we many international students. We have an instance where a CTR was required and passport was used as ID. Item 14e (issued by) was not completed. I'm assuming this is an exception, but not really sure. Are passports issued by specific countries?

Passport Identification Pages:
All passports, either in the front or back, have identification pages, usually facing, which contain at a minimum,
(1)holder's picture,
(2)holder's signature,
(3)country of issue,
(4)holder's permanent address at time of issuance,
(5)date of issuance,
(6)date of expiration.
Some passports have all this information on the same page as the holder's picture.
Some students should also have a Class H-1B Student visa that indicates that they are a non-US Student enrolled in degree-granting program. Be sure that these are not expired.Remember that some of the 9/11 hijackers were here on student visas!
