Okay, say we have a home where the backyard is located in a SFHA. No part of the house is located in the SFHA. Is flood insurace required? My first thought is no, but I wanted to check with others first.

If no part of the structure is in the flood zone then no insurance is required. Page 23 of the Guidelines addresses this.

I agree with CF.
Your determination may however not split hairs...If that's the case, your provider should have a procedure for locating the improvements in relation to the SFHA.

Can the bank check the flood map and document the position of the structure vs. the SFHA or is the flood vendor the only one that can do it?

There is no requirement that you use a vendor at all. If you want to maintain a full time staff to monitor all changes, you can do it yourself.

That being said, paying a vendor makes the most sense. Their contract should require them to investigate the occasional deal where only part of the land is in a SFHA.

Cowboy Fan, I searched the fema website for the guidelines and couldn't find anything specifically mentioning what we are discussing. Is there any way you can direct me to where I need to go. Thanks.

If the bank has used a vendor for determinations, the bank cannot review a map and change a determination. The procedure in this type of situation is generally to provide a copy of a plot plan or other document that shows the placement of the structure to the flood vendor. They overlay it on the flood map and determine its placement vs the flood zone and reissue as necessary, generally for no added charge.
