If a customer who had previously signed up for e-statements and shown demonstrable consent changes their e-mail address, do you need to prove demonstrable consent again for the new e-mail address (same customer and account)?

No. There is no such requirement for a change of e-address.

Demonstrable consent would be required when you have a substantive change such as in systems or the method of delivery, such as changing from an encrypted PDF to something else. In this case, it is a common email address. That shouldn't be "substantive" in my book.
