HUD published the wording in the 3/26/07 issue of the Federal Register. Here's a link to the document. http://a257.g.akamaitech.net/7/257/2422/01jan20071800/edocket.access.gpo.gov/2007/pdf/E7-5412.pdf

The wording is slightly different than the wording on a document posted on HUD's web site with a 4/30/07 OMB expiration date. Here's a link to that document. http://hudclips.org/sub_nonhud/cgi/pdfforms/92070.pdf
