If we have two different branches (and therefore two different accounts) of a listed company, do we just file one DEP form? In the past, we have filed two DEP's with corporate addresses and in the DBA section, we put the city name and state store (i.e. Rock Island, IL store). Do we just file one form instead? Thanks.

If you have a single tax identification number, all that is needed with the other qualifiers is a single DEP. It is the same business.
