Husband comes in with two checks to cash and it triggers a CTR. One of the checks was payable to the wife, she signed and then he signed it.

Do we have to fill out two section A's since one check was to her, but he was the only one that came in? I think just him, but have thought about it too much.

I would assume just him, but I don't know. I have already demonstrated my CTR incometency.

I would show the husband in Section A of page 1, check box e in Section B and then show the wife in Section A on page 2.

I concur with devsfan, I would show both the husband and wife as benefactors and check "conducted on own behalf" for transactor.

How do you know the wife is a benefactor? Once she signed the check it is a bearer instrument, and anyone (in this case the husband) could benefit from it. If she was with him and received the cash, yes she would benefit from it. But in this case, she may never see it. If you include her on the CTR, you are assuming she does. I'd personally file on just him.

I agree with CJT. Unless you have some knowledge that she benefited from the cash, the wife should not be listed on the CTR.
