Am I correct in my thinking that a Bank is required to use the standard HUD-1 Statement that contains "U.S. Department of Housing and Urban Development"? I have run into some situations where Banks have created their own version of a settlement statement. I think the regulation is pretty clear in the definition of HUD-1 Settlement statement in that it is the document as prescribed by the Secretary.

Within the regulation there is a list of items that you are permitted to change on the sample form. I don't have the reg in front of me to see if this would be permitted or not.
