A required deposit perhaps? In what context is it used?

I feel like this is twenty questions.

Well, I FINALLY found out what it was about -people need to not truncate things so much. It was a Credit REPORT consent form for depositors. A bank with accounts from an opt-in state wanted to know if other local banks had adopted this practice. I had the feeling it was simple, but never would have gotten it from the original 4-word description.

Thanks!

Where the heart is
If you need a consent form to obtain credit reports on authorized signers, there is such a form in BOL Bankers Tools:
Credit Report Consent Form
