If an consumer application has joint at top of form and then also in the middle of the form has joint, endorser, guarantor information and the box is check correctly in both areas, can a bank still be wrote up for not suffice information for joint intent?

The examples in Regulation B indicate where the applicants initial next to the checked joint intent box and not just have the box checked. Whether or not you have a violation is questionable without the initials, but it would result in a best practice recommendation by me.
