Do we use the new form to register MSBs now or do we still have to use the old form? I am hearing conflicting things.

According to the MSB website the new registration form is not to be used until 9/01/2008.

You should not be using the form - the MSB should be using the form. I see banks complete these forms for their MSBs - and IMHO - that is a very dangerous practice.

I was not meaning that I would be filling out the form. Of course the MSB would be filling out the form.
