How many of you require that DBA's provide you with documentation that they have properly "noticed" their fictitious name before opening a new account?

We require some proof of the business' name. If the owner's last name isn't part of the business' name, we require a Fictitious Name Statement (so does the State). Otherwise, we'll take a Business License or Seller's Permit.

Ditto - in KY, DBAs not using the last name in the title must also file such notice to the sec. of state. We require this registration. We too use the business license for verification.
