Remember you don't insure the dirt, just the improvements where there are 2 walls and a roof.

Not required -- but it is a good idea to do a determination before any improvements begin. If the borrower just wants to own a vacant lot, there is no need to worry about flood. But if they are planning to build something, I think it is better to know now -- before the purchase -- whether it is in a flood hazard zone.
