Due to amount of originations we fall under the small reported. For the field that reads "ethnicity of applicant or borrower collected on the basis of visual observation or surname" are we to enter the fields for N/A on both?

Reporting the ethnicity of an applicant is the same whether or not you are a small reporter. Therefore, you would not report N/A on that field just because you are a small reporter.

If your borrower is an entity, your responses to collected on the basis of visual observation would be Not Applicable. If the application was taken for one or more individuals on a face-to-face basis with your loan officer, and the borrower filled out the DI information. then your answer would be NOT collected on the basis of visual observation. If the application WAS face to face, and the borrower did not provide the information, and the loan officer completed the ethnicity etc information based on visual observation, then your response would be yes, it was collected on the basis of visual observation.
