Does anyone know where I can get a sample fraud/forgery investigation form? (Q&A) We have one but it is slightly outdated....a lot of repeat questions.

I got the Fraud Affidavit from Bankers Tools...Thanks!.... we had a question and answer form for the customer...what, where, when, how did it happen...is that really necessary? Ours had a lot of repeat questions on it which I probably could get rid of.
