Has anyone else seen error resolution procedures that include providing provisional credit within 5 days but then reversing the provisional credit if written notification is not received from the customer within 10 days? I realize that provisional credit does not have to be provided if written notification is not received within 10 days of requesting it. However, I can't find anything that states one way or the other about reversing the credit for not receiving the notice. Would appreciate any guidance or opinions.

I believe the reason no one has replied to this is because no one does this. You can make an argument that Reg E requires written notification to get the provisional credit, so if you don't get the written notification, you don't have to give the provisional credit. However, VISA requires provisional credit within 5 days and doesn't require a written notification. If you reversed the credit at 10 days, you would be fine with Reg E, but you would have violated VISA's rules.
