Is there a time period in which a terminated employee must receive information pertaining to their 401K plan balance and their options?

I'm not sure about a time frame, however it seems customary when an employee's separation packet (COBRA, exit interview, etc)is sent that 401k information is included. Possibly check with the 401k holding trustee. There may be limitations set forth in the agreement.
