Each time we do a new loan for a customer, the system generates a"W-9 REQUEST FOR TAXPAYER ID AND CERTIFICATION". Must we have a new form with each loan or is one form good for a year or a certain amount of time.?

I think it's a good idea to do it each loan, it just makes tehm verify and certify their tax id number. Not sure if they ever expire as the number should never change for an individual, business or DBA's would be a little more tricky.

I agree it is a good idea to obtain new certification for each loan (or renewal), but do we need to keep prior copies, or just the most recent?
