Mr. Smith comes into the bank and deposits $15,000 in cash into his joint account with Mrs. Smith. I know I should complete two Section A's - one for Mr. and one for Mrs., but what about Section B. Do I check "Conducted on own behalf" even though it wasn't both Mr. and Mrs. that actually made the deposit? Or do I complete Section B with Mr. Smith's information since he was the one that made the deposit?

Put Mr. Smith in Section A on the front of the form, and Mrs. Smith in Section A on the reverse.

Check "conducted on own behalf" in Section B on the front of the form. That indicates that Mr. Smith conducted the transaction. Don't list his information a second time.
