Owner of a LLC company came in and made a withdrawal of $18,000.00. I know I list the company and the owner as Parties. My question is, Pretty sure the company's involvement would be "person on whose behalf transaction is conducted'. Not sure what to put for the owner... would it be "person conducting transaction on own behalf'?

The owner would be conductor on behalf of another.

Teller said that customer never stated one way or the other. Just made the withdrawal.

No. Unless the customer readily admits what they need the money for in conversation, we don't ask.

New thought. If the transactor can be A and B in involvement, aren't we suppose to check A?

The new requirements state that if a transactor is both and A and B you include them as both with the corresponding transactions listed under each part. I hope that explanation makes sense.
