If John makes an $11,000 cash deposit into an LLC account and a $500 cash deposit into his joint account he has with his wife, will there be 4 Part 1's? One for the LLC (2c marked $11,000), one for the wife (2c marked for $500), one for John (2b marked and include both acct numbers and $11,500 for the amount) and another for John (2a marked for $500)?

There will be four part 1s. But not quite as you've described them.

John, 2a and 3 checked, $500 with the joint account number
John, 2b and 3 checked, $11,000, with the LLC account
Wife, 2c checked, $500 with the joint account number
LLC, 2c checked, $11,000, with the LLC account number

Deposits by a conductor to his joint account only have the 2a designation. You don't do a 2b. The second Part 1 for a joint account deposit by an owner is the 2c for the joint owner. That's all. The combination of the two part 1s with a common account number shown indicates it's a deposit to a joint account owned by the two identified individuals.
