ALL,

I have a transaction that should have been documented on a systematic MIL. Unfortunately, since the teller did not run the transaction properly, the system did not prompt for the MIL data. So, does anyone have a manual form they are willing to provide to assist me?

We created an excel spreadsheet that resides on the server. We limited the access to only those who needed the information and the ability to enter information only to those who needed it.
It is working very well. It has been very helpful with the new MLR needs.
