If a customer is opening multiple accounts on the same day and all of the information on the CBO form is the same, can we use one form for all accounts, or must we have different forms to 'prove' verification for each account?

This question has been answered a few times in different ways. Some banks are entering all the account numbers on one form. Others are photocopying the form for each account and entering the account number on it. Consider how you will be maintaining the records afterwards in making your decision

Once you have made your decision, communicate it and make sure it is followed consistently.
