my customer is a church.
it is registered with the state as a NON-PROFIT CORPORATION (per papers on file)
do I need the CBO with control prong only???

so I think I found the answer
only need the control prong if the nonprofit had to file with secretary of state
so in our case YES we should have completed the CBO
