If a customer wants to add a joint owner to his/her deposit account agreemnt (signature card), would the date of the signature card be dated the date of revision or would the original account opening date be on the card?

The date of the revision...you need to be able to demonstrate who the authorized signers were as of any specific date over the life of the account.
