If a customer calls and has the bank employee do a transfer is that considered a telephonic call counting against reg d limit?

Yes.

I always heard it referred to it as the "Rear Rule." If you have to get off your rear to do it, then it isn't Reg D. For example, ATM or going to a teller line. But calling in on the phone would be considered a Reg D transaction, even if a teller performs the transaction.
