I told him that we have to have a specific reason for denial.
This was for a customer to renegotiate rate of current loan with us.

Doesn't sound like you ever took an application if this is your standard response to anyone that wants to negotiate existing rates. However, that statement is not a reason, if not.

Actually this was for a mortgage loan and they wanted to reduce their rate. They did complete an application for the request.

And the loan was with us already for a few years.

The you owe them a AAN and the statement made by the loan officer is not a reason.
