How do I report a co-signer (not a co-borrower) on the LAR?

The same as a co-borrower if they signed the note as individuals.

The note specificly states the borrow with borrower's signature and then "co-signer" with co-signer's signature. (front and back) The application was not signed by the co-signer by she is listed on the front of the app with name, soc. sec., address, etc. Still report the so-signer the same as borrower?

Yes. If their income was used in the credit decision you would also include it in the income reported.
