Without regard to advisability (which would depend on an institutions risk profile), is anyone aware of a regulatory REQUIREMENT that an institution perform an annual Fair Lending Review/Audit?

No. But I sure like to have the reviews to give to the examiners. They use mine for both their fair lending and CRA exams, and it cuts down on the amount of time they stay in the bank.

I use the same exam procedures that our examiners use.
