Just kidding. Because there are liability issues involved with your decision to allow the customer to not receive their statement in the normal manner, I wouldn't feel comfortable giving you ours to rely on.

Check with your document provider - they should have some form of "no mail request" that you could use, with approval of your counsel, of course.
