I am reviewing our "Annual Billing Notice" before we have them printed and mailed. We want to mail out one disclosure to all of our HELOCs and open end lines of credit that are not secured by real estate. If that is the case which Model form do we use? G-3 or G-4???

I would like to know this answer also and pose another question...on these annual billing notices, if we do not offer any credit card products, can we omit the credit card verbiage from the model notices?

I believe you would have to use G-3 for HELOCs and G-3a for all other open-end loans.

G-4 (and G-4a) is one of the short-forms that you would have on the back of your statement, rather than sending an annual statement.

On the annual billing notices, if we do not offer any credit card products, can we omit the credit card verbiage from the model notices?

Seems stupid to disclose something that does not apply to your bank.
