Maximum Rate Schedule

I'm fairly new to SC and understand that there is a disclosure we must post that indicates the maximum interest rates the bank charges. Can anyone direct me to the SC statutes that indicates what this disclosure must say? Would anyone be willing to share a copy of their disclosure?
