I just noticed our current notice does not contain the expiration date, but does contain all other information verbatim. Is that a requirement?

OMB Approved documents cannot be altered unless there are specific provisions that allow it - i.e., highly unusual.

There is a line on this form under "How does a Servicemember or Dependent Request..." that calls for the insertion of bank info.

Actually it appears that the only change from the prior version are the dates in the header and footer of the disclosure, so a little editing of the current document should be all that's needed.

I just noticed that the word doc and PDF on BOL has expiration of 03/31/2020 while the one from the HUD has 03/31/2021

I have updated ours to the 2021 date. Why am I not surprised they changed it. HUD has been notorious in the past for revising this document several times until it finally gets it right. The first iteration of the form did state 2020.

I thought effective date was December 31, 2019, the SCRA expanded foreclosure protections will sunset, and the protection period will revert to the period of active duty plus 90 days. With this new HUD SCRA notice, the sunset date will be 2021? Or am I mixing two different dates?

Did anyone notice that the HUD Form has a typo in the very last paragraph? It reads: "(toll free from the Unites States)"
So do we correct this to read "United" States or, in light of Randy's valid point, leave it alone?

That typo isn't new and rather than appear unprofessional, it is corrected in the Word version. I cannot imagine being criticized for that correction of an obvious error.

When I look at the notice on the HUD website, it looks like they corrected "Unites" -- but the BOL pdf version looks like it still has the typo?

I can only assume they were reading this thread and fixed it as the PDF form was downloaded based on the GAO newly released form.
The PDF can't really be used so I doubt I'll replace it. Since your bank info needs to be inserted the PDF is more so a go-by and we already give you a Word version.

Actually - you are required to send it to all home loan borrowers prior to them going 45 days past due - service member or not.
Does anybody know why the model form on the HUD site shows an expiration date of 3.31.2021? I have not come across anything saying it was extended beyond 12/31/2019.

That date is the OMB expiration date. The OMB expiration date has nothing to do with the timing of the wording of the notice regarding the "one year" or "90 days" time frame.

The National Defense Authorization Act for Fiscal Year 2018 (H.R. 2810). Section 557 of the Act extends for two years, until December 31, 2019, the sunset on the temporary change of 90 days in § 303(b) and (c) of the SCRA (50 U.S.C. § 3953) back to one year.
In my mind that would be the expiration date as that foreclosure extension has been a key part of that form but the thought may be that it can be cancelled sooner. I've no idea where the new date came from. It could be moot as S2155 will make that 1-year a permanent change. The OneSource contract could also change or contact info. I assume that is a periodic contract for financial counseling that is occasionally put out for bid.

OMB expiration dates are usually for 3 years. It doesn't mean it will or won't be compliant for 3 years. It just means they need to get a new OMB approval in 3 years.

https://legalassistance.law.af.mil/ does work and is the best alternative, even when I do get a "non-trusted site" message. The DMDC was that way before as the military appears to give site certificates little attention.
Since we know the given URL is bad, this is the same site. It's certainly a low-risk issue.
