Found this:
Directions
Mix 1 cup grated smoked mozzarella, 1/2 cup grated parmesan, 8 ounces cream cheese, 1 minced garlic clove, 1 box thawed frozen spinach, 1 jar artichoke hearts, and salt and pepper in a slow cooker. Cover and cook on high, 2 hours.
And here's a more complicated version:
2 (8 ounce) packages cream cheese, softened
3/4 cup half-and-half
1 tablespoon onion, finely chopped
1 garlic clove, minced
1 (10 ounce) bag frozen chopped spinach, thawed and drained
1 (13 ounce) can quartered artichoke hearts, rinsed drained and chopped
1/2 cup parmesan cheese, shredded
2/3 cup monterey jack cheese, shredded
Directions:
1 Combine cream cheese and half and half in a bowl until well blended.
2 Add all ingredients except monterey jack and stir well.
3 Pour mixture into crock pot, cook on high for 1 1/2 to 2 hours or until warmed through.
4 Sprinkle with monterey jack and enjoy!

Not a crock pot recipe, but I don't see why it wouldn't work:
1 brick cream cheese, room temp
1 big spoonful of mayo (I usually sub sour cream)
1 bag (about 6-8 oz, I think) shredded Italian cheese blend
2 tsp minced garlic or garlic paste
pinch dried basil (or sub Italian herb blend)
1 can artichoke hearts, rinsed, drained, and chopped
1 small box frozen chopped spinach, thawed and drained
big handful shredded mozzarella (probably about 1/2 cup or so)
The usual process is to stir together everything except the mozzarella, dump into a small buttered casserole dish, top with the mozzarella, and then bake at 350 for 20-30 minutes.
