CTR Question

I have looked thru posts, but did not find what I am needing. My questions is about filling our a CTR that involves a husband and wife. Husband has a DBA account and wife is authorized signer and they also have a joint account. Wife comes in and deposits 9k cash to the DBA account and 5K cash to the joint account. Would I show 14K cash for husband (account owner-on behalf of) an wife 9k cash (conductor on behalf of another) 5k cash (on own behalf)? I have never come across this and would appreciate any help. If I missed the post, please direct me to it. Thanks

List both accounts

Husband
Check 2(c)
Item 21 is $14,000
List both accounts

Do not make multiple entries for the same person.
