If I have a loan that only modified the maturity date and nothing else would I report it on my CRA LAR as long as that loan had not been reported in the same calendar year?
Yes. For CRA modifications that extend maturity are reportable.

Would the same apply if you have a rate reduction, but the the maturity date remains the same?

No, it is my understanding only an extension of maturity.
Correct; only modifications that extend maturity (or increase amount) would be reportable.
