How to Report CRA Small Bus. for One Time Close

For One-time Close (OTC), when the loan is booked the call code is 01A2 and therefor not picked up for CRA small bus reporting, however when the loan goes into permanent phase and the call code is changed to 01E1/01E2 we are looking for an understanding of how to report these on the CRA Small Bus LR. For the action taken date, is it the date of the promissory note, or is the action taken date the date the call code changed? Also, if a loan originates in one year but doesn't change to a reportable call code until a later year is this one reported, or do you only report if the origination and the call code change occurs in the same calendar year?
