Reg E and Provisional Credit

What procedures does your institution follow before giving customers provisional credit when filing a dispute? What type of documentation do you get, etc.....

Most banks will ask for a completed affidavit and/or police report. However, remember you can not require them to do these things before you proceed with the investigation.
