Reg CC Large Deposit hold

If a hold requires part of a deposit to be release in a couple of days and the rest later, how do you accommodate the different amounts and dates on a hold notice. Do you have a single notice with multiple amount and dates or do you produce multiple notices.

I've seen both approaches used, but if I had a choice, I would use just one piece of paper.
