If borrower has a Fraud alert statement from back in 2004 but the number on the alert is now disconnected - what would be recommended for documentation?

I'd suggest following up with the customer and letting them know it's still out there and that the number shown is incorrect. Then verify them according to your internal procedures before you proceed with any loan requests. We would document in the file that we contacted and verified the borrower.
