HMDA Data Collection internal form

HMDA Data Collection internal form - 05/01/17 10:02 PM

Does anyone have a HMDA Data Collection form they would be willing to share? I just want to compare it to the one we are using. We are new to HMDA and 2016 was our first year of collecting data and reporting by 3/1/17. We tweaked it several times throughout the year, but am wondering if there is something better laid out than what we have.
