New Product/Service Risk Assessment

Hello all! I'm looking for a risk assessment template that I can use for documenting the risks associated with new products or services being considered. This would be completed during the evaluation phase, before final approval. If anyone has something similar and you are willing to share, I'd greatly appreciate it!
