Reg. E Error Resolution & Preauthorized Transfers

Can the A-3 Model Form (error resolution notice on periodic statements) satisfy the requirement to have a phone number for preauthorized transfers (§ 1005.9(b)(6)) / § 1005.10(a)(1)(iii))?

Or should there be a separate statement that says "direct inquires to"?

It is my understanding that the intent is to have the number clearly available. So while I would certainly use the model form, I would also ensure it is readily apparent that inquiries should be directed to (800) 555-1234.
