On real estate loan applications are we required to use the 1003 uniform residential loan application?

No, there is no mandatory use requirement, especially if you portfolio your loans. However, each of the regulators has requirements for obtaining certain pieces of information for loan accounts. You may want to check your regulator to ensure that you collect everything you need.
