Back in March 2014 there was an amendment to TN Public Chapter No. 597 to allow TN banks the ability to cash small checks without requiring that an estate account be opened.
Does anyone have a form that they are using for this? There are several pieces of information that we are required to obtain and I am trying to come up with a form we can use in these situations.

We created an affidavit for this purpose, however, there are times when it has to be revised to meet a particular situation. If you want to provide your personal email, I will send it to you.

The provision was enrolled as 45-2-711. Developing an affidavit to be executed under penalties of perjury would not be difficult, but it would be better if your attorney did it.
