What is the required verbage that should be contained in an agreement with a customer regarding sending information via secured e-mail?
Answer: 

There is no "required" verbiage. But I used to have a canned response something to the effect of this:

Email is inherently unsecure. It is similar to sending a postcard through the mail. Having your confidential information (deleted in this response) sent in an unsecured manner greatly increases your chance of falling victim to identity theft. Additionally, we cannot verify that the sender of the email is authentic and we cannot respond to this message with confidential account information. We would instead refer you to our toll free line, xxx-xxx-xxxx or to log in through our secured internet banking web site where this information may be exchanged.
