Looking for BSA Risk Assessment Procedures

After a recent BSA Audit, the FDIC has required us to complete a BSA Risk Assessment. We requested info on a format and/or procedure for conducting such an assessment in the BOL Banker's Threads. There has been a lot of activity - but it all has been of other bankers requesting the same help. Is there a tried and true format or procedure to use?
