MC Require Notarized Affidavit for DC Fraud?

Does Mastercard require a notarized affidavit for debit card fraud and if the answer is no, when did it go away ?
Answer: 

I've been processing MasterCard claims for nine years and a notarized affidavit has never been required during that time. MasterCard either requires a signed cardholder letter (not an affidavit) or an Expedited Billing Dispute Resolution Process (Fraud) Form completed by the issuer certifying the cardholder's claim.
