Member FDIC & Outhouse Logo on Survey Form

If we send surveys out to consumers requesting information on deposit and lending products, do we need to have Member FDIC and the Outhouse logo on the survey form?
Answer: 

If the mailing consists only of a survey, and there are no marketing materials included, you are probably not dealing with an advertisement at all. Therefore, there is no need to include the Equal Housing logo or message. If your bank name appears on the survey, I'd tend toward the conservative and include "Member FDIC" on the form. If any advertising is enclosed with the survey, you should include the appropriate advertising statement or logo, depending on the content of the ads.
