Question: 
I need a Sample Hold Harmless Agreement so that we can issue two Debit Cards to non-signers on a Church Business Account. With this Agreement we would not be held liable for any unauthorized transactions that should arise. If no one has a sample, can someone tell me what verbiage should be included in a hold harmless agreement?
Answer: 

What you are proposing is not a standard banking relationship. It will require a unique document drafted by your attorney. That fact alone should give your bank some reason to reconsider whether it is advisable.
