Signatures Required on Safe Deposit Lease

Does the deputy listed on the safe deposit lease also need to sign the entry card? In our quarterly audit we found a card that was not signed. Is this correct or will we be dinged in an audit for not having this signed?
Answer: 

A deputy does need to sign the entry card initially when he/she is appointed deputy and should sign each time he/she enters the box. The deputy does not have to sign the contract because he/she is not an owner.
